Doctor witnessed for the first time in 22 years: 350 grams of prostate removed by closed method

SAMSUN (İHA) – A citizen who has been suffering from prostate disease for many years had a 350-gram prostate removed through a closed method. Associate Professor Dr. Önder Çinar, who has been practicing medicine for 22 years, stated that it was the first time they removed such a large prostate through a closed method and emphasized that the case is rare in medical history. For many years, 64-year-old Ilhan Tanrıver, who had prostate disease and was afraid to see a doctor, applied to the hospital when his prostate reached an unbearable size. Tanrıver, who did not want to undergo open surgery, could not have his 350-gram prostate treated with the closed method in Ordu. After conducting research, Tanrıver contacted Associate Professor Dr. Önder Çinar from the Medicana Health Group, and with the diagnosis of his benign prostate, he got rid of almost half a kilogram of prostate through the closed method known as holmium laser enucleation of the prostate (HoLEP).
“350 grams of prostate is the preference for ‘open surgery’ for most physicians” Urology Specialist Associate Professor Dr. Önder Çinar from Medicana International Samsun Hospital, who mentioned that a prostate close to half a kilogram should be treated with open surgery, said, “Ilhan came to our outpatient clinic with a complaint of ‘urinary retention’ with a catheter inserted. He had been suffering from prostate disease for a long time but had not received treatment. After the tests, we saw that the patient’s PSA (Prostate-Specific Antigen) levels were high. Therefore, we conducted all tests including MRI. We detected a ‘benign prostate’ in him. We planned his surgery to ensure that he urinates comfortably. Normally, a 350-gram prostate is the preference for ‘open surgery’ for most physicians. Nowadays, with technological advancements, especially with the laser technology, we can completely remove the prostate by entering through the urethra, a procedure known as HoLEP, without any bleeding or pain occurring in the patient. With this surgery, the patient does not have to stay in the hospital for a long time. Usually, the patient, who is monitored for one night, is discharged.”
“A prostate close to half a kilogram had never been removed through a closed method before” Emphasizing that the closed method surgery they applied for the case is very important in terms of medicine, Associate Professor Dr. Çinar said, “When our patient came to us, he had a 350-gram prostate. We discussed all treatment options with him. We also discussed open surgery, but he preferred endoscopic closed surgery. Since we performed closed surgery, there was no need for the patient to have a catheter for a long time, to have a drain, or to be hospitalized. It has been 2 days since the surgery, and we will discharge our patient in good health. The prostate enlarges with age. We expect those over the age of 50 who have difficulty urinating to have regular check-ups. Prostate can be detected in the early stages during annual check-ups. Ilhan’s problem was related to urinary retention. There was also blood in his urine. We performed a successful surgery. There were no problems after the surgery. I graduated from medical school in 2002. I have been a physician for 22 years. I have been a specialist since 2012. I have been in the urology community for 16 years. There have been cases where we performed surgery for a 350-gram prostate through open surgery. Maybe we encounter 1-2 patients whom we perform open surgery on in a year. I don’t remember a closed surgery of this size being performed in our region. I think our case is the first case in which a 350-gram prostate was removed through a closed method in the prostate operations performed so far. It is possible to perform closed surgery on large prostates regardless of the size of the prostate. HoLEP surgery has an equal success rate with other surgical methods. Therefore, I would like to remind our patients with large prostates who are afraid of surgery that surgery can be performed through a closed method.”

Associate Professor Dr. Önder Çinar added that it is vital for men, especially from middle age onwards, to undergo prostate examinations at least once a year in order to prevent prostates from growing too large.
